Payment is due at time of service.
Payment is due at time of service. Clients are expected to make payment arrangements with billing department prior to visit.
Cancellation of sessions
Cancellation of sessions with less than 24 hours notice incurs a fee of: $100.00
Session Duration
Sessions are limited to 45-50 minutes; extra time is charged accordingly at $200 hourly.
In-network and Out-of-network
We are in-network with Penn Behavioral Medicine, Aetna, UnitedHealthCare (UBH), Highmark, Horizon, AmeriHealth, Federal Employee Program and Medicare. We are out of Network on Keystone HMO, Cigna, Personal Choice and all other plans.
Account delinquency
Accounts delinquent greater than 90 days will accrue 15% (APR) interest compounded monthly.
Prior to session
Client must contact insurance carrier to determine deductible, co-insurance or copay and any preauthorization requirements.
Requests for records, reports and forms
Must be submitted at least 7 days prior to deadline and may be subject to an administrative fee.
